IMPORTANT NOTICE REGARDING “NO-SHOWS”
AT THE LOUIS LASKY MEMORIAL MEDICAL & DENTAL CENTER

Dear Member:
Due to the increasing number of people who fail to keep their scheduled appointments at the Louis Lasky Memorial Medical & Dental Center in Lindenhurst, we have instituted a $25 “no-show” penalty fee per appointment missed without notification. This change is effective January 1, 2012 and applies to members and their eligible dependents. Should an appointment need to be cancelled or changed, we require at least twenty-four (24) hours notice.
A failed appointment affects many people: (1) the doctor or dentist who was prepared to see you; (2) another patient who could have been seen; (3) you, since you did not receive the medical/dental care you need, and (4) all the medical doctors and dentists, including the support staff, who have to be paid whether you show up or not. Failed appointments are a giant waste of time and money.
We have tried many different ways to deter this inconsiderate behavior. We make phone calls to confirm appointments. We suspend members who are chronic offenders. We send letters to the chronic offenders to let them know what their no-show means to the Center. Yet too many of our members still make appointments and just do not show up for them.
It is very easy to avoid the no-show $25 penalty.
1. If you make an appointment, then show up.
2. If you or your eligible dependent cannot keep your scheduled appointment at the Center, you must call at least twenty-four (24) hours in advance to cancel your appointment. This small courtesy will enable the staff at the Center to attempt to fill your open appointment with another patient who needs it.
Please note that any patient who fails to show for an appointment will not be permitted to make another appointment unless the $25 penalty is paid.
